Mobilizing for Action through Planning and Partnerships (MAPP)
Be Well Placer, a partnership facilitated by Placer County Public Health and informed by members of the community, utilizes a nationally recognized public health planning model called Mobilizing for Action through Planning and Partnership (MAPP). This community-wide strategic planning process guides the partnership by applying strategic thinking to prioritize heath issues, identify resources to address them, and then take action. MAPP is an interactive process that can improve the effectiveness, efficiency, and the performance of the entire local public health system. The vision of Be Well Placer is a Placer without barriers that fosters collaboration and opportunities where the health, welfare, and safety of everyone is promoted, enhanced, and protected.
The MAPP process includes the six phases illustrated in the figure above. We are pleased to share the documents from the third phase, the Four MAPP Assessments, below:
- The Community Themes and Strengths Assessment measures the community’s perceptions through broad community involvement. This assessment provides a deep understanding of the issues that residents feel are important through community meetings, stakeholder focus groups, and data collection through surveys.
- The Community Health Status Assessment summarizes priority community health and quality of life issues such as “What does the health status of our community look like?” and “How healthy are our residents?”
- The Local Public Health System Assessment focuses on all of the organizations and entities that contribute to the public''s health.
- The Forces of Change Assessment identifies culture shifts, trends, and internal and external forces that may impact the community’s health in the future. It focuses on both positive and negative forces and opportunities.

Data contained in the four assessments were examined by the Placer Partnership for Public Health to identify the key issues that must be addressed in order to realize the shared vision. Goals were set for each issue and strategies formulated for achieving them. These goals and strategies, published in the Community Health Improvement Plan, map the route from the current circumstances of the community to the future laid out in the vision.
Next will come the planning, implementation, and evaluation of the action that the group takes to achieve its goals.The process is described as a cycle because it is ongoing, as action is continually evaluated and adjusted to achieve greater effectiveness.
